Required Documents
- Social Security Cards: Provide copies of cards for each client listed on the application.
- Income verification: Provide copies of all income from the last 30 days for everyone on the application 18 years or older.
- Social Security, SSI, APA, TANF, food stamps, or general assistance recipients: Send a copy of your most recent award letter.
- Food stamps recipients: THRHA can use a recent award letter that shows countable income to determine your eligibility.
- Self-employed clients or household members: Send all pages of your most current income tax return. If you or a family member claim zero income, you must complete a zero-income waiver as part of the application process
Your application may be delayed or denied if you do not submit all required documents.
